Program Information

Diabetes affects a cross-section of people and is precipitated by a variety of circumstances.

One-size-fits-all care plans disregard the unique needs of each patient, which is why the Signature Diabetes Institute has developed a variety of diabetes management programs. The different plans are further personalized based on the needs of the individual patient. Our plans help us construct a framework for your care, on which we can build a personalized, cost-effective plan of care that suits your individual needs.

SDI Programs

Duration: 12 Months

One – Prevent Diabetes

First degree relatives of people with diabetes or people with prediabetes will gain appropriate knowledge and access to ongoing support to implement changes to postpone onset of diabetes.

Two – New Onset Diabetes

Patient diagnosed with diabetes within 6 months prior to referral. Obtain appropriate knowledge of disease state & lifestyle efforts needed to achieve & stay in control, optimize medications, with future access to ongoing care via program four if metabolic targets achieved. If not, patients can access program three at a discounted rate.

Three – Uncontrolled Diabetes

Patient with diabetes and an A1c over 8%. Obtain appropriate knowledge of disease state & lifestyle efforts needed to achieve & stay in control, have an A1c under 7% (if clinically appropriate), medications will be optimized, with future access to ongoing care via program four if metabolic target achieved. If not, patients can access program three at a discounted rate (up to 2 cycles).

Four – Diabetes Control Maintenance

Patient who has achieved metabolic targets, will continue to receive ongoing education, motivational support, medication management, explore opportunities to reduce medications and screening for complications.

Five – Gestational Diabetes (-insulin)

Patient will be educated to achieve and maintain blood glucose within target (without insulin therapy) for the duration of pregnancy & will receive postpartum education to maintain euglycemia or near euglycemia. Patients may then elect to follow either program one, three or four on an ongoing basis, depending on level of metabolic control after delivery.

Six – Diabetes in Pregnancy (+insulin)

Patient will be educated to achieve and maintain blood glucose within target (with insulin therapy) for the duration of pregnancy & will receive postpartum education to maintain optimal glucose control. Patients may then elect to follow either program three or four on an ongoing basis, depending level of metabolic control.
